Every Sofwave Treatment That's FDA-Cleared, and the Evidence

BY DR. SIMA SHAHBANDAR6 MIN READREGENERATIVE AESTHETICSSKIN HEALTH

You've probably had the moment. You catch your reflection in a car window or on a video call, and you notice the softness that's crept in along the jaw, or the little crepiness under the chin that wasn't there a few years ago. Someone mentions "ultrasound skin tightening," a friend brings up Sofwave, and suddenly you're reading marketing pages that all sound the same and promise a great deal.

Here's the part that rarely makes it into those pages: Sofwave is not one treatment. It's a single device that has earned a handful of separate FDA clearances, each for a specific concern, each backed by its own study. Knowing which is which is the difference between an informed decision and a hopeful one.

I like to walk patients through the actual clearance list before anything else, because the honest map of what a technology is proven to do is more useful than any before-and-after wall. So let's do that here.

What Sofwave is actually cleared to treat

The FDA doesn't clear a device in the abstract. It clears it for named uses, in named populations. For Sofwave, the SofWave System was first cleared in November 2021 as a non-invasive aesthetic treatment to improve facial lines and wrinkles, to lift the eyebrow, and to lift lax submental (beneath the chin) and neck tissue, in people aged 22 and older (FDA 510(k) K211483).

Since then the list has grown, one indication at a time. There's a clearance for short-term improvement in the appearance of cellulite, granted in December 2022. There's one for the treatment of acne scars, from August 2023. And there's one for improvement of the appearance of skin laxity on the upper arms, from December 2023.

That "short-term" wording on cellulite is not a slip. It's the language of the clearance itself, and we repeat it exactly, because it tells you something true about what the data supports.

How it works, without the hype

The technology behind Sofwave is called SUPERB, short for Synchronous Ultrasound Parallel Beam. In plain terms, the handpiece delivers seven parallel, non-focused beams of ultrasound energy into the mid-dermis, at a depth of roughly 1.5 millimeters, warming that layer to about 60 to 70 degrees Celsius. At the same time, a cooling plate keeps the surface of your skin protected. The warmth in the mid-dermis is what prompts your body to remodel collagen over the following weeks.

One point worth clearing up, because it gets muddled constantly: Sofwave is not HIFU, and it does not target the SMAS, the deeper muscular layer that a facelift addresses. It works in the skin, not beneath it. That's not a weakness. It's simply a different tool for a different job, and knowing the difference helps you match the treatment to what's actually bothering you.

Why the number of sessions depends on what you're treating

This is where a lot of online summaries get sloppy, flattening everything into "one treatment." The studies don't support that shorthand. For the face and neck, for cellulite, and for the arms, the registrational protocols used two sessions. For acne scars, the pivotal study used three. The single-session data comes specifically from the facial fine-lines work, and it doesn't transfer to the other indications.

My view is that the number of sessions isn't a menu you pick from, it's a clinical judgment based on the concern, the tissue, and what the evidence for that concern actually showed. When a plan is built around the data for your specific indication rather than a one-size promise, you know what you're signing up for.

What the research shows, and where it stops

The study numbers are genuinely encouraging, and they're also specific. In the facial trial behind the 2021 clearance, 80 subjects were treated across 467 facial areas, and 80 percent showed improvement by 12 weeks. In the acne-scar study, 67 subjects at four U.S. sites received three treatments, and 97 percent showed improvement, with an average reduction of about 46 percent from their baseline scar severity. In the cellulite study, blinded reviewers correctly picked out the post-treatment photo 89 percent of the time at the three-month mark.

Two honest caveats belong right next to those figures. First, on durability: the published follow-up for these treatments is measured in months, not years, and we won't tell you a result is guaranteed to last longer than the evidence has actually looked. Second, on skin tone: no pigmentary adverse events were reported in published studies including Fitzpatrick I to IV and Asian skin. That's a careful, accurate statement, and it's the right one to make rather than a blanket claim that any device is safe for absolutely everyone.

What this means for you

Picture someone in Lubbock in her early fifties. She isn't chasing a different face. She'd just like the line of her jaw to read the way it does in her own memory, and she's wary of anything that sounds like a sales pitch. What she needs isn't a promise. It's a physician who can look at her skin, tell her which of Sofwave's clearances actually fits her concern, explain how many sessions the evidence for that concern supports, and say plainly what the research does and doesn't yet show about how long it holds.

That conversation is the real value. The device is only as good as the judgment deciding when, where, and whether to use it, and on whom. For some people Sofwave is a genuinely good match. For others, a different approach fits better, and hearing that honestly is worth more than hearing yes.
